APPLICATION & WAIVER

All guests must submit a complete New Client Registration PRIOR TO STAY WITH US. Our Enrollment packet includes Application, Medical History, Medical Care Release, Policies & Liability Waiver, Personality Profile, & Muli Pet Boarding Waiver.

VACCINATIONS:

All dogs must have up-to-date vaccinations.
Owners must submit written proof from their veterinarian:
• DHLPP or DAPP (distemper and parvo)
• Bordetella (annually)
• Rabies is required for dogs over 4 months of age
• Canine Influenza – VERY STRONGLY ENCOURAGED

FEEDINGS

In order to minimize upset tummies, we strongly recommend you bring your pet’s food from home while boarding. You should pack more food than normal for each meal during longer stays, especially for active dogs. Please pack food into ziplock bags or containers that will fit into their cubby as storage space is limited. You are welcome to bring your pups favorite treats or chews as well.

MEDICATIONS & SUPPLEMENTS

If your dog is taking any medications or supplements that they will need while you are away, please complete a medication form and leave the medication with us in the original container. It will be placed in a secure location and given as requested.
